LPN - Yonkers NY

Conduct advanced clinical home care services, including patient assessment, medication management, and patient/family education. Perform revisits to patients based on skilled needs, implement care plans, and evaluate treatment progress. Accurately observe, measure, record, and report clinical data, including documenting symptoms indicating health decline. Reconcile and administer prescribed medications, and perform nursing treatments such as urinary catheterization and sterile dressing changes. Document visits electronically within 24-48 hours under the direction of an RN. Requires NY State LPN license, one year Med/surg experience, and good computer skills.
