Job Summary
Provide professional nursing care for residents of all ages in long-term care under the supervision of a registered nurse or physician. Demonstrate competency in administering medications, performing assessments, and responding to emergent situations while ensuring resident health, comfort, and safety. Document resident interactions, care outcomes, and dietary needs; counsel families on prevention and treatment plans. Work as a member of an interdisciplinary care team, maintaining effective relationships with providers and the public. Available for PRN shifts including weekends and holidays, with weekly hours varying.
Required Qualifications
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse
- Associate degree in practical nursing
- Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum
- Minimum of 400 hours of experience as LPN at another facility
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board
- Possess multi-state licensure privileges
- Obtains and subsequently maintains required department specific competencies and certifications
- Completes required annual continuing education units (CEU) and competency training requirements of the department and facility
- Must be available to work 6 shifts per month
- Must be available to work two weekend shifts
- Must be available to work holiday rotation
- Must pass a drug screen
- Must pass pre-employment background screening
Desired Qualifications
- Associate degree in practical nursing is preferred
