Job Summary
Assess patients, manage medications, and implement care plans for homebound individuals recovering from illness or surgery. Perform skilled nursing treatments including catheterization and sterile dressing changes, while documenting visits electronically within 24-48 hours. Monitor and report clinical data, communicating any signs of health decline to the care team. Requires NY State LPN license, one year of med-surg experience, a driver's license, and strong computer skills.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a driver license and car
- Good computer skills
