LPN - White Plains NY

Conduct advanced clinical home care services for patients recovering from illness, injury, or surgery, including patient assessment, medication management, and patient/family education. Perform revisits to patients based on skilled needs, implementing the plan of care and evaluating treatment outcomes. Accurately observe, measure, record, and report clinical data related to health status, while documenting visits electronically within 24-48 hours. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Collect and communicate signs of health decline promptly. Requires NY State LPN license, one year med/surg experience, and a driver's license.
