Job Summary

Record a patient's medical history accurately, take vital measurements, and observe patients under treatment to identify progress and side effects. Monitor fluid intake and output, compose patient charts, and assist with daily activities such as eating and bathing. Administer injections, prescribed medications, enemas, wound care, and collect samples for lab testing as assigned. Provide emotional support, communicate with families regarding advice and comfort, and supervise Certified Nursing Assistants in accordance with state regulations. Requires a current LPN license, CPR/BLS certification, and experience in long-term care geriatric populations.

Required Qualifications

  • Graduate of an accredited Practical Nursing program.
  • Current, unrestricted LPN/LVN license in the state of practice.
  • Knowledge of long-term care regulations, resident rights, and skilled nursing facility standards.
  • Experience providing direct patient care to geriatric and long-term care populations.
  • Ability to administer medications, treatments, and physician-ordered therapies safely and accurately.
  • Skilled in monitoring residents' conditions and reporting changes to the interdisciplinary care team.
  • Experience with electronic health records (EHRs) and clinical documentation.
  • Knowledge of infection prevention and control practices.
  • Strong assessment, communication, and critical-thinking skills.
  • Ability to supervise and delegate tasks to Certified Nursing Assistants (CNAs) in accordance with state regulations.
  • Ability to work effectively with residents, families, physicians, and interdisciplinary team members.
  • Compassionate, resident-focused approach to care with a commitment to preserving dignity and quality of life.
  • Current CPR/BLS certification.
  • Background check and pre-employment drug screen required.
