Job Summary
Conduct home care visits across Lorain, Medina, and Western Cuyahoga Counties at $35 per point, with travel reimbursement paid per mile. Maintain electronic medical documentation and participate in an on-call rotation approximately every three to four weeks. Weekend coverage occurs every three to four weeks, with one to two holidays annually. Full-time and part-time positions are available; full-time staff receive PTO and 401k, while part-time employees are also eligible for PTO.
Required Qualifications
- LPN license in state of Ohio
- Electronic Medical documentation experience
- Willing to travel throughout your designated area
- Must participate in On Call rotation (approx. every 3-4 weeks)
