LPN - West New York NJ

Map out and oversee patient recovery processes from illness, injury, or surgery at home by providing advanced clinical services including patient assessment, medication management, and family education. Perform skilled revisits to patients based on clinical need, implement care plans, and evaluate treatment outcomes. Accurately observe, measure, record, and report clinical data while documenting visits electronically within 24-48 hours. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Collect and communicate signs of health decline immediately. Requires NY State LPN license, one year med/surg experience, valid driver's license, car, and strong computer skills.
