LPN/Welness Director

Direct and oversee memory care resident care services in the community; develop, implement, and update individualized service plans; conduct resident assessments and monitor changes in condition; review medication administration practices and documentation; recruit, hire, train, supervise, and lead the care staff; provide coaching and staff development; ensure staffing schedules meet care needs; ensure regulatory compliance and accurate clinical documentation; participate in state surveys, inspections, and audits; implement policies for quality assurance and infection control; collaborate with Executive Director and department leads to ensure comprehensive resident care; demonstrate strong leadership, communication, and organizational skills to support a positive, collaborative team culture.
