LPN - VSG Willingboro

Collaborate with practice leadership to conduct daily huddles and weekly care team meetings reviewing population health data to improve care and achieve practice goals. Support the delivery of patient care by preparing patients for exams, administering medications and vaccines, performing phlebotomy, and conducting diagnostic tests such as EKGs and pulmonary function tests. Coordinate test and referral tracking, lead patient education efforts, and support care management through self-management training for chronic conditions. Under clinician direction, ensure outreach to patients within 48 hours of hospital discharge or seven days of ED visits to facilitate transitions of care. Maintain par levels of clinical supplies and perform daily regulatory testing to ensure equipment accuracy and compliance. Requires a current NJ LPN license, CPR certification, and minimum six months of experience in a physician practice.
