Job Summary
Licensed Practical Nurse (LPN) Visit Nurse providing in-home direct patient care per plan of care, conducting skilled assessments, developing and implementing care plans, documenting per agency/Medicare guidelines, educating patients and families, and collaborating with providers to adjust care as patient needs evolve. Must maintain nursing scope of practice, perform 1:1 patient care, contribute to reducing readmissions through preventative care, and work within an equal-opportunity, patient-centric home health team. Requires LPN license in the applicable state(s), CPR/BLS/First Aid, reliable transportation, and at least 2 years of home health experience. Includes flexible scheduling (FT/PT/PRN) and benefits with Interim HealthCare.
Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license as an LPN/LVN in the state(s) in which he/she practices
- At least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Two (2) years of experience practicing as an LPN in a home health or similar setting
