LPN Visit Nurse

Job Summary
Provide in-home direct patient care according to the provider's prescribed plan of care while closely monitoring progress and recovery. Complete skilled assessments, implement comprehensive care plans, and communicate patient conditions to collaborate with providers. Ensure accurate clinical documentation in accordance with agency protocol and Medicare guidelines. Provide education to patients and family members and work to decrease readmissions by promoting preventative care. Maintain current LPN licensure, CPR/AED/BLS/First Aid certification, and at least 1200 hours of experience or completion of the preceptor program.
Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license as an LPN/LVN in the state(s) in which he/she practices
- At least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program
- Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Two (2) years of experience practicing as a Licensed Practical Nurse (LPN in a home health or similar setting
