LPN Visit Nurse / Hospice - PRN

Assess, treat, and communicate patient needs with speed and clinical precision as a vital support member of the interdisciplinary care team. Deliver professional nursing care across the full nursing process, conducting thorough patient and family assessments to contribute to individualized care plans. Complete all visit types as needed, administering medications and treatments per physician orders while identifying nursing diagnoses from health assessment data. Record accurate, timely clinical notes regarding pain and symptom management outcomes, and communicate changes in patient condition to physicians. Coordinate with community health partners to support seamless care transitions and ensure necessary equipment and services are arranged. Maintain and grow clinical skills through continuing education and in-service participation. This role requires an adaptable RN with strong assessment skills and sound clinical judgment under pressure, working PRN across Springfield and surrounding areas.
