Job Summary
Conduct patient assessments, manage medication administration, and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Implement and evaluate individualized plans of care for patients recovering from illness, injury, or surgery in their homes. Document visits electronically within 24-48 hours and report clinical data regarding patient health status or symptom decline. Requires NY State LPN license, one year of med/surg experience, and strong computer skills.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Good computer skills
