Job Summary
Provide direct patient care including monitoring vital signs, administering medications, and assisting with wound care. Collaborate with registered nurses and physicians to develop and implement individualized patient care plans. Document patient information accurately in medical records to ensure continuity of care. Assist patients with activities of daily living such as bathing, dressing, and mobility support. Observe and report changes in patient conditions promptly to the healthcare team to facilitate timely interventions.
Required Qualifications
- Current and valid LPN license in the state of employment
- High school diploma or equivalent; completion of an accredited practical nursing program
- Basic Life Support (BLS) certification or equivalent
- Demonstrated ability to perform clinical nursing tasks safely and effectively
- Strong communication skills and ability to work collaboratively within a multidisciplinary team

- Experience in hospital, long-term care, or outpatient clinical settings
- IV therapy
