Job Summary
Oversee clinical care and daily operations of an assigned nursing unit while supervising nursing staff, assistants, and support personnel. Monitor resident conditions, review assessments, and ensure care plans are updated and implemented appropriately. Coordinate communication with physicians, nurse practitioners, residents, and families; review laboratory results, provider orders, incident reports, and changes in resident condition. Participate in care plan meetings, QAPI activities, family meetings, and interdisciplinary team discussions. Assist with admissions, discharges, transfers, and coordination of outside appointments. Ensure unit supplies, documentation, and resident records are accurate and current. Support staff education, orientation, infection prevention efforts, and quality improvement initiatives. Address resident, family, and staff concerns while promoting a positive care environment. Located in Cheektowaga, NY, this full-time days shift role offers a sign-on bonus up to $3,000 and pays between $28 and $32 per hour.
Required Qualifications
- High School Diploma or equivalent
- Valid NYS Licensed Practical Nurse (LPN) License
- Minimum 1 year of LPN experience in a long-term care setting
- Strong knowledge of resident care planning, clinical documentation, and long-term care regulations
- Leadership experience with the ability to supervise, coach, and mentor staff
- Excellent communication, organizational, and problem-solving skills
- Ability to coordinate care across multiple disciplines and manage competing priorities
- Commitment to providing compassionate, resident-centered care
