LPN - Union City NJ

Conduct advanced clinical home care services including patient assessment, medication administration, and nursing treatments such as urinary catheterization and sterile dressing changes. Perform patient revisits based on skilled needs, implement the plan of care, and evaluate treatment outcomes. Document visits electronically within 24-48 hours and report clinical data regarding patient health status. Accurately collect and communicate signs of health decline under RN direction. Requires NY State LPN license, one year of med/surg experience, a driver's license, and good computer skills.
