Job Summary
Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education under RN direction. Perform revisits to patients in their homes based on skilled need, implementing care plans and evaluating treatment progress. Accurately observe, measure, record, and report clinical data, while documenting visits electronically within 24-48 hours. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes. Collect and communicate any signs or symptoms indicating a decline in health status. Requires NY State LPN license, one year med/surg experience, driver's license, and good computer skills.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a driver license and car
- Good computer skills
