Job Summary
Licensed Practical Nurse (LPN) on the TRU PACE interdisciplinary team providing nursing care to participants and assisting the Registered Nurse with implementing the Plan of Care. Works in home health/hospice settings across Boulder/Denver area; promotes participant goals, meets Medicare/Medicaid conditions, and contributes to all clinical services. Responsibilities include data collection on body systems and vitals, integrating participant preferences into care, accurate timely documentation, participant and caregiver education on topics such as medications, safety, nutrition, and overall health; supports team decisions, demonstrates effective communication, participates in on-call rotations (nights, weekends, holidays), and may assist with procedures and immunizations as needed. Requires travel to multiple locations, valid driver’s license with adequate insurance, and CPR certification (or within 60 days). CPR/IV/phlebotomy skills preferred; computer literacy and agency technology use required.
Required Qualifications
- Must possess a current LPN license in the state of Colorado
- Minimum one year LPN experience – hospice or home health experience preferred
- Experience working with frail/elderly preferred
- Experience working with individuals with mental health needs desirable
- Bilingual in Spanish preferred
- Must successfully pass agency required background checks and drug screening
- Must possess a valid driver’s license, provide proof of insurance, and have reliable transportation
- Requires travel to multiple work locations
- Must possess CPR certification or be able to obtain within 60 days of hire
- Must comply with agency health screening and immunization requirements
- Must be able to demonstrate computer competency
- IV certification and phlebotomy skills preferred
- Must be able to demonstrate computer competency
