Job Summary
LPN Triage Nurse will process incoming calls from families, patients and others, identify caller needs, update the plan of care with physicians and the hospice team, and provide education and supportive care to patients and caregivers. Responsibilities include ordering DME for new admits and current patients, assisting in implementing improved work methods for compliance with laws and policies, supporting the hospice team’s care coordination, coordinating and communicating daily RN Case Manager schedules, participating in quality assessment/Performance improvement activities, collaborating with pharmacies and DME vendors, facilitating delivery or maintenance of medical equipment, ordering testing and imaging as needed, submitting real-time documentation to promote continuity of care, attending required meetings, and assisting with Volunteer Physician management. Required qualifications include a Louisiana LPN license, completion of an accredited practical nursing program, minimum 3 years LPN experience (hospice preferred), and CPR/BLS; a valid Louisiana driver’s license is also required.
Required Qualifications
- Valid Louisiana Licensed Practical Nurse (LPN)
- Education: High School Diploma or equivalent; Completion of an approved, accredited practical nursing program.
- Experience: Minimum three (3) years’ experience as an LPN; Hospice experience preferred.
- Possesses valid Louisiana Driver’s License, Automobile Liability Coverage, BLS CPR cert.
