LPN Triage GI

Respond to patient calls and messages to triage requests for clinical advice, care plans, prescription refills, test results, prior authorizations, and forms completion. Collaborate with GI Fellows and providers to schedule hospital follow-up visits and procedures, while refilling medications per protocol and obtaining insurance authorizations. Enter and coordinate provider orders for medications, labs, imaging, and DME, monitoring outstanding items and verifying results. Perform care tasks including immunizations, injections, phlebotomy, wound care, EKGs, and x-rays if certified, and administer screening tests for hearing and vision. Review charts for upcoming appointments, retrieve pending results, and document chronic and population health management services. Provide patient education, assist with exams, and offer cross-floor coverage as needed. Work full-time Monday through Friday from 8am to 5pm.
