Job Summary
Licensed Practical Nurse (LPN) on the Transition Team delivering quality home-care nursing to infants, children, and adults and supporting the transition of care from hospital/HICU to ongoing BAYADA home care services. Partners with CHOP and our Transition Team to care for clients with rare and complex diseases, while providing extensive training to the ongoing BAYADA care team. Present openings include overnight shifts and weekend day shifts, with full-time or part-time scheduling. Responsibilities include delivering skilled nursing care, coordinating with hospital teams, and educating the home-care team to ensure smooth transitions. Certifications to advance skills, ongoing paid training, comprehensive benefits, PTO, 401(k), and performance-based compensation are provided. Requires travel throughout Philadelphia County and a current LPN license, with NICU/PICU/ICU or home-care trach/vent experience and CPR certification.
Required Qualifications
- Have a current license as a Licensed Practical Nurse
- NICU, PICU, ICU, or home care trach/vent experience required
- Achieve a passing score on high-tech Adult and Pediatric simulations
- Hold current CPR certification
- Ability to work in a teaching setting to prepare and educate the ongoing BAYADA care team
- Availability to commit to an ongoing, consistent schedule
- Ability to travel throughout Philadelphia County
