Job Summary

Provide nursing care in accordance with resident assessments, care plans, and facility policies. Administer medications, treatments, enteral feedings, and IV therapies within the LPN scope of practice. Monitor resident conditions and promptly communicate changes to providers, supervisors, and responsible parties. Accurately document care, treatments, observations, and medication administration. Supervise and support Nursing Assistants to ensure quality resident care. Maintain up-to-date resident care information and communicate pertinent information during shift changes. Follow infection control, safety, resident rights, and confidentiality standards. Participate in interdisciplinary care planning and quality improvement initiatives. Assist with admissions, transfers, and other nursing responsibilities as assigned.

Required Qualifications

  • High School Diploma
  • Valid NYS Licensed Practical Nurse (LPN) License
  • Current CPR Certification
