Monitor resident health and wellbeing through medication administration, vital signs collection, and clinical assessments via Personalized Service Plans. Document clinical care changes, complete monthly wellness visits, and communicate service updates to the Wellness Director and families. Provide direct resident care as needed while maintaining clinical quality assurance standards. Work overnight shifts from 6pm to 6am, with flexibility for weekends and peak service times.
