LPN - Teaneck NJ

Conduct advanced clinical home care services, including patient assessment, medication management, and patient/family education. Perform revisits to patients based on skilled need, implementing the plan of care and evaluating treatment outcomes. Document visits electronically within 24-48 hours and accurately record clinical data regarding patient health status. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Collect and communicate signs of health decline per physician plans. Requires NY State LPN license, one year med/surg experience, driver's license, and good computer skills.
