Job Summary
Maintain consistent embedded presence at DaVita clinics while executing core daily care coordination tasks such as prescription management, lab orders, referral coordination, and post-discharge follow-up. Directly oversee a team of 8–12 Embedded Care Coordinators by providing daily guidance, coaching, and performance monitoring against KPIs. Serve as the primary escalation resource for the team, backfill for CCs as needed, and travel to facilities to ensure operational continuity. Support onboarding and training of new staff, ensuring completion of all required credentialing modules. Monitor weekly clinical and operational metrics, flag workflow gaps, and propose solutions to refine CC-facing tools and SOPs.
Required Qualifications
- CMA or LPN license
- Valid Driver's license and access to a vehicle
- At least 2 years of management experience in a healthcare setting
- Knowledge of principles and processes for providing customer and patient services, including customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction
- Knowledge of business and management principles involved in strategic planning, resource allocation, human resources modeling, leadership technique, production methods, and coordination of people and resources
- Proficiency in the structure and content of the English language, including the meaning and spelling of words, rules of composition, and grammar
- Understanding of principles and methods for curriculum and training design, teaching and instruction for individuals and groups, and the measurement of training effects
- Familiarity with principles and procedures for personnel management
Desired Qualifications
- Experience in a lead, senior, or supervisory care coordination role
- Familiarity with facility-based operations and workflows
