Job Summary
Overnight LPN/LVN providing direct resident care in a skilled nursing setting. Responsible for supervising non-licensed team members, coordinating residents' health and wellness needs, and collaborating with family members and community physicians to ensure personalized care. Responsibilities include ensuring quality assurance and regulatory compliance, overseeing resident care coordination, and maintaining open communication with families and physicians. Must have a graduate nursing program background and current NJ Practical Nurse/Licensure, with at least one year of experience in long-term care or similar settings. Knowledge of RAI and the Care Plan Process, plus strong organizational, problem-solving, and computer skills (MS Office) are required.
Required Qualifications
- NJ LPN/LVN license
- Graduate of approved college/school of nursing
- Current state license as Practical Nurse/Vocational Nurse
- Minimum of one year experience in long term care, assisted living, skilled nursing, hospital, and/or full service operations
- Knowledge of nursing practices, techniques and methods as applied to skilled nursing and resident care
- Knowledge of federal, state and local long term care regulations
- Understanding of the Resident Assessment Instrument (RAI) and the Care Plan Process
- Ability to handle multiple priorities
- Written and verbal communication skills
- Organizational and time management skills
- Good judgment, problem solving and decision making skills
- Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel)
