LPN - Staten Island NY

Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education under RN direction. Perform skilled patient revisits, implement care plans, and evaluate treatment progress while documenting visits electronically within 24-48 hours. Accurately observe, measure, record, and report clinical data, including identifying and communicating signs of health decline. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes. Requires NY State LPN license, one year med/surg experience, and good computer skills.
