LPN - Stamford CT

Conduct skilled home care visits for patients recovering from illness, injury, or surgery under the direction of an RN or Coordinator of Care. Perform patient assessments, administer prescribed medications, and execute nursing treatments such as urinary catheterization and sterile dressing changes. Reconcile care plans, evaluate treatment progress, and document visits electronically within 24-48 hours. Accurately observe, measure, and report clinical data, including symptoms indicating health decline. Requires NY State LPN license, one year of med/surg experience, a driver's license, and good computer skills.
