LPN Staff, 5-Star Hospital Based Skilled Nursing Unit. Full Time Days

Provide bedside nursing care, including checking vital signs, monitoring intake/output, and assisting with activities of daily living. Administer oral, topical, and injectable medications; perform wound care, dressing changes, and catheter insertions. Observe and document changes in patient condition, reporting significant findings immediately to the supervising RN or physician. Assist in the implementation and revision of individualized care plans; ensure all treatments align with physician orders. Teach patients and families about disease management, medication requirements, and post-discharge care. Oversee and delegate tasks to Certified Nursing Assistants (CNAs) to ensure comprehensive unit care. Maintain accurate, real-time records of vitals, treatments, and patient progress. Full Time Days, 36 hrs. per week, 12 hr. shifts.
