Job Summary
On-site Behavioral Health Coordinator (LPN) in Sarasota supporting a Behavioral Health Integration program across post-acute facilities. Manages a caseload, conducts monthly behavioral health screenings, identifies and escalates changes in patient condition to the BHCM or clinical team, and coordinates additional behavioral health services. Serves as the primary on-site behavioral health contact for assigned facilities, collaborates with nursing staff, therapy teams, social services, and IDT members, and maintains accurate monthly documentation in the EHR while adhering to privacy and workflow standards. Requires travel between facilities within the regional territory, strong organizational skills, proficiency with EHR/digital tools, cultural competency, and the ability to work independently in clinical settings.
Required Qualifications
- Experience working in healthcare, behavioral health, care coordination, case management, or related patient-facing work
- Ability to manage a high-volume caseload and navigate multiple facility environments
- Proficiency with electronic health records (EHR) and digital communication tools
- Strong communication, organization, critical thinking, and follow-through
- Demonstrated cultural competency and ability to work effectively with diverse populations
- Reliable transportation and ability to travel within a defined region
