Job Summary
On-site behavioral health coordination for patients in post-acute facilities within Lakeland/Wauchula regions. Manages a caseload, conducts monthly behavioral health screenings, and escalates changes in condition to the BHCM or psychiatric providers. Serves as primary on-site behavioral health coordination contact with facility leadership, nursing, therapy, social services, and IDT teams; supports scheduling and on-site logistics for psychiatric/psychology services. Maintains accurate monthly documentation in the EHR, adheres to privacy/compliance standards, and follows established workflows. Requires healthcare/behavioral health experience, strong organizational and communication skills, ability to work independently in the field, proficiency with EHR/digital tools, cultural competency, and travel within a regional territory.
Required Qualifications
- Experience working in healthcare, behavioral health, care coordination, case management, or related patient-facing work.
- Ability to manage a high-volume caseload and navigate multiple facility environments.
- Proficiency with electronic health records (EHR) and digital communication tools.
- Strong communication, organization, critical thinking, and follow-through.
- Demonstrated cultural competency and ability to work effectively with diverse populations.
- Reliable transportation and ability to travel within a defined region.
