Job Summary
Provides direct patient care as defined by the State Nurse Practice Act and hospice care plan. Implements current nursing practice per comprehensive assessment and plan of care, documents patient services accurately to reflect the plan, and educates patients and family/caregivers about diagnoses and self-care. Coordinates hospice services with an interdisciplinary team, reporting identified needs (e.g., aides, OT, PT, MSW, Dietitian, Clinical Supervisor) and manages pain/symptom control while evaluating treatment responses. Requires a graduate of an accredited practical/vocational nursing program with a current state license and CPR certification; driver’s license with insured vehicle. Schedule is Monday–Friday, 8am–5pm.
Required Qualifications
- Licensed Practical Nurse (LPN) with current state license
- CPR certification
- Valid driver's license with automobile insured accordingly
Desired Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Current LPN license to practice in the state
- CPR certification
- Must be a licensed driver with an automobile insured as required
- Home health or hospice experience preferred
