Job Summary
Conduct patient assessments, administer prescribed medications, and perform nursing treatments such as urinary catheterization and sterile dressing changes for home care patients. Implement plans of care, evaluate treatment progress, and document visits electronically within 24-48 hours while reporting clinical data and health status changes to an RN. Reconcile medications and perform skilled revisits based on patient needs under RN direction. Requires NY State LPN license, one year of med/surg experience, a driver's license, and car.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a driver license and car
- Good computer skills
