Job Summary
Conduct patient assessments, administer prescribed medications, and perform nursing treatments such as urinary catheterization and sterile dressing changes for home care patients. Revisit patients based on skilled needs, implement care plans, and evaluate treatment progress under the direction of a Coordinator of Care RN. Document visits electronically within 24-48 hours and report clinical data regarding health status or symptom declines. Requires a NY State LPN License, one year of med/surg experience, a driver's license, and strong computer skills.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a driver license and car
- Good computer skills
