Job Summary
LPN Intake Coordinator role involves facilitating patient scheduling across multiple disciplines as directed by management, coordinating approvals for additional visits, processing physician orders and lab results between physicians, homecare partners, and patients/caregivers, and ensuring timely communication and documentation to support care coordination within a homecare setting. Strong customer service, phone and communication skills, and computer literacy are required; current LPN license in the agency’s state (preferred) and valid driver’s license with reliable transportation are also needed. The role focuses on coordinating orders, lab results, and multidisciplinary communications to support managed care patients within the NHC HomeCare environment.
Required Qualifications
- High school education / GED required
- Two years’ experience in home care or related health care role required
- Education/experience in computers required
- Driver’s license and reliable transportation required
- LPN license preferred in state of agency
- Excellent customer service, phone and communication skills required
