Conduct skilled home care visits for patients recovering from illness, injury, or surgery, including patient assessment, medication administration, and clinical treatments like catheterization and sterile dressing changes. Reconcile prescribed medications and implement the physician's plan of care under RN direction. Perform revisits based on skilled needs, evaluate patient treatment progress, and document visits electronically within 24-48 hours. Accurately observe, measure, record, and report clinical data regarding health status, including any signs of decline outside established parameters. Maintain a driver's license and car for travel, with a minimum of one year med/surg experience and a valid NY State LPN license required.
