Job Summary
Conduct patient assessment, medication management, and patient/family education for home care recovery. Perform revisits to patients based on skilled need, implementing the plan of care and evaluating treatment outcomes. Document visits electronically within 24-48 hours and record clinical data regarding health status. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Monitor for signs of health decline and communicate symptoms immediately. Requires NY State LPN license, one year Med/surg experience, driver's license, and computer skills.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a driver license and car
- Good computer skills
