Job Summary
Licensed Practical Nurse (LPN) needed to provide direct nursing care and support to residents in Salem Transitional Care. Responsibilities include administering medications and treatments, monitoring resident conditions, assisting with assessments and care planning alongside the RN team, completing charting and documentation, supporting residents and families with professionalism and compassion, assisting with admissions/discharges, responding to emergencies and providing CPR, and helping oversee medication administration when applicable. Ideal candidates demonstrate strong clinical and critical thinking skills, teamwork, dependable attendance, excellent communication, and a patient-centered approach. Six months of long-term care experience is preferred. Required Qualifications
