Job Summary
Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education under RN direction. Perform revisits to patients based on skilled needs, implement care plans, and evaluate treatment outcomes. Document visits electronically within 24-48 hours and record clinical data regarding health status. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes. Maintain accurate documentation of any symptoms indicating a decline in patient health. Requires NY State LPN license, one year med/surg experience, valid driver's license, and strong computer skills.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a driver license and car
- Good computer skills
