LPN - Rockville Centre NY

Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education under RN direction. Perform revisits to patients in their homes based on skilled need, implementing the plan of care and evaluating treatment outcomes. Accurately observe, measure, record, and report clinical data related to health status, while documenting visits electronically within 24-48 hours. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes per physician plans. Requires NY State LPN license, one year med/surg experience, driver's license, and good computer skills.
