Job Summary
Provide in-home nursing care to pediatric patients across Delaware County. Responsibilities include delivering clinically appropriate nursing services within scope of licensure, maintaining updated patient records, evaluating nursing care effectiveness, consulting with physicians on care plan changes, mentoring peers during orientation, educating patients and caregivers, participating in QA/QPI activities, and adhering to compliance and ethics standards. The role requires travel to patients’ homes, flexible schedules with 8-, 10-, or 12-hour shifts, and availability across days of the week. Required Qualifications
- LPN/LVN Nursing Degree
- Unencumbered LPN/LVN License in state of employment
- 1 year of demonstrated nursing experience
- Valid driver license with proof of insurance
- Clear background screening
- CPR certification (in-person hands-on skills check)
- Hep B screening
- TB screening (2-step or 1-step with 2-step history)
- Working knowledge of Nurse Practice Acts
- CPR card/certificate
- Proficiency in English (verbal and written)
- Ability to travel to patients' homes
- CPR and safety training
- Enhanced Nurse Licensure Compact (where recognized)
