Job Summary
Licensed Practical Nurse (LPN) delivering in-home pediatric nursing care. Responsibilities include providing care within licensure scope, maintaining updated clinical records, evaluating nursing service effectiveness, consulting with physicians on care plan alterations, mentoring/new employees during orientations, educating patients and caregivers, participating in QAPI, attending mandatory trainings, submitting personnel documentation timely, maintaining patient confidentiality, and performing duties across home-based visits with travel to patients' homes. Requires travel to patients' homes; shifts may vary (8/10/12 hours) and may include any day of the week. Required Qualifications
- LPN/LVN Nursing Degree
- Unencumbered LPN/LVN License in the state of employment or Multi-State license through the Enhanced Nurse Licensure Compact
- 1 year of demonstrated experience
- Valid driver license with proof of insurance in applicable states
- Clear background screening
- Valid and current CPR card/certificate with in person instructor led hands on skills check
- Clear Hep B Screening in applicable states
- Clear TB Screening in applicable states
