Job Summary
Conduct on-going patient care and assessments for infants, children, and adolescents in their home settings while administering prescribed medications and therapies. Focus on the medical needs and treatment plans provided by physicians, coordinate care, and educate family members to enhance positive outcomes. Implement preventative initiatives to protect quality of care and maintain a 1:1 patient care ratio. Work full-time or part-time shifts including days, nights, and weekends with flexible scheduling options.
Required Qualifications
- Active RN or LPN/LVN license
- Ability to provide care in a client home setting
- Full-time and part-time shifts (days, nights and weekends available)
- Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care
- Conduct on-going patient care and assessments
- Administration of prescribed medication, treatments, and therapies
- Coordination of care
- Educate family members on patient clinical care to enhance positive outcomes
- Preventative initiatives to protect quality of care for patient
