Job Summary
Conduct on-going patient care and assessments for infants, children, and adolescents in their home settings according to the active plan of care. Administer prescribed medications, treatments, and therapies while focusing on medical needs and treatment as directed by physicians. Coordinate care and educate family members on clinical care to enhance positive outcomes, alongside implementing preventative initiatives to protect quality of care. Perform 1:1 patient care with flexible scheduling available in days, nights, and weekends.
Required Qualifications
- Active RN or LPN/LVN license
- Provide care in a client home setting
- Must be available for full-time and part-time shifts (days, nights and weekends)
