Job Summary
Conduct on-going patient care and assessments for infants, children, and adolescents in home settings while administering prescribed medications and therapies. Focus strictly on the active plan of care provided by physicians to coordinate care and educate family members on clinical procedures. Implement preventative initiatives to protect quality of care and maintain a 1:1 patient care ratio during night shifts. Provide direct, one-on-one medical support to the medically fragile community with 24/7 clinical support available.
Required Qualifications
- Active RN or LPN/LVN license
- Provide care in a client home setting
