LPN / RN - Outpatient Addiction Treatment Nurse

Job Summary
LPN or RN needed in Portland, OR for outpatient addiction treatment clinic to administer medications per physician orders, support MAT for opioid use disorder, maintain documentation and EMR records, and coordinate care with the treatment team while ensuring HIPAA compliance and CARF standards. Responsibilities include vital signs, medication inventory, patient assessments, TB screening as applicable, front-office support, and contributing to a recovery-focused environment. Requires Oregon nursing license and at least one year of direct patient care experience.
Required Qualifications
- Active LPN or RN license in the State of Oregon
- Minimum one year of direct patient care, nursing, or clinical experience
- Knowledge of medication administration, patient assessment, and nursing best practices
- Strong communication and patient engagement skills
- Basic computer proficiency (Microsoft Word, Outlook, Excel)
- Ability to maintain accurate clinical documentation and medication records
- Experience with outpatient nursing, ambulatory care, or MAT is preferred
