LPN/RN Nurse Supervisor | Evening Shift (3 PM–11 PM)

Provide direct nursing care in accordance with physician orders and individualized care plans, administering medications, completing assessments, and performing wound care while maintaining accurate documentation. Supervise, support, and mentor nursing staff and Certified Nursing Assistants to ensure compliance with state, federal, and facility regulations. Monitor resident conditions, communicate changes to physicians and family members, and participate in quality improvement, infection prevention, and survey readiness initiatives. Collaborate with the interdisciplinary team to ensure exceptional resident outcomes and participate in an on-call rotation as needed.
