Job Summary
Administer medication, perform client treatments, and prepare clinical notes while monitoring and documenting changes in patient conditions. Encourage social and rehabilitative skills to promote self-care, independence, and established goals within the Plan of Care. Collaborate with the clinical team to develop and improve care plans, maintain aseptic technique, and participate in client care meetings and staff training. Work every other Monday and Friday shifts in the Tri-County area of Ohio, providing skilled nursing and personal care in home settings. Maintain licensure, CEUs, and mandatory employment requirements while adhering to HIPAA guidelines and agency policies.
Required Qualifications
- Valid State LPN License
- Valid driver's license
- use of automobile
- Annual MUI (Incident) training
- ability to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures
- ability to use a computer, laptop or tablet
- ability to think independently, recognize potential problems, gather information, and know when to contact a supervisor
- ability to promote excellent customer service and always provide exceptional client care
