LPN/RN 1:1 - 2026/2027

Provide individualized healthcare support to a student with specific medical needs within the Francis Wyman Elementary school setting. Accompany the student to and from school while riding the bus, and assist with mobility, feeding, toileting, positioning, or other personal care needs. Execute the student's Individualized Health Care Plan, Emergency Action Plan, and medical directives, including administering medications and treatments per physician orders. Monitor acute and chronic conditions, respond to medical emergencies, and perform specialized procedures such as tracheostomy care and gastrostomy tube feedings. Maintain accurate health records, collaborate with school staff and families, and ensure all services comply with local, state, and federal regulations. Maintain current LPN/RN licensure in MA and CPR certification.
