Job Summary
Deliver compassionate, high-quality care to patients across the lifespan while partnering with providers to ensure seamless, coordinated treatment. Support patients and families in understanding health needs, document care accurately using the EHR system, and participate in care huddles and team-based quality improvement initiatives as a key member of the Patient-Centered Health Home model. Requires an active LPN/RMA license in North Carolina, accredited program graduation, and current CPR/BLS certification. Bilingual candidates are preferred. Required Qualifications
- LPN / RMA License OR Certification, active and in good standing with the State of North Carolina
- Graduate of an accredited program
- Highschool graduated/GED
- Current CPR/BLS certification
- Valid Driver's License
Desired Qualifications
- One (1) year of clinical experience in a primary care, community health, or outpatient setting
- Experience with underserved or rural populations
- Familiarity with team-based care or PCMH/PCHH models
- Bilingual
